PPM Failed Proficiency Testing

I am new in my POCC position, so I'm a little lost as to how to handle this. I have a group of providers in one of my on-site clinics who perform wet preps and KOH wet preps. They get PT three times a year via API's virtual images. The performing providers have failed the last three PPM events. The first one was for the KOH, and the second two were for regular wet preps.
