CAP GEN.43825 - Result Verification

I feel like I am having a pre-CAP brain cramp, but this question is confusing to me. It says "Manual and automated result entries are verified before final acceptance and reporting by the computer"


So in our lab, we have an elaborate process of manual entry verification where a second tech prints the manually entered results and checks them for accuracy before the first tech can release them to the chart. We don't do that for POC tests that are manually entered, such as urine pregs and rapid streps....am I over thinking this? How are you meeting this at your site?

  No, you're not having a brain cramp, as we struggled with this issue too Smile.  We too have an elaborate process in our lab about verifying manual entries; second tech checks entries (though it's AFTER the results are entered), then signs off that they were 'verified.'


  Talk about your nightmare for POC, yes.  Right or wrong, I interpreted the CAP standard to mean results that generate an instrument printout that are then entered manually.  That said, I got any and all POC devices interfaced right quick so there was virtually no manual entry!  I made the judgement call that the standard does not apply to a manual test without a printout where, essentially, there is nothing to "verify", without, say, speaking to the person that entered it! (not do-able).  If an inspector would challenge anyone on this standard on a manual POC test, well then I would love to hear what he or she would suggest be done.  Right or wrong, that's where I left it for my facility.

A hospital in our system was recently cited for this by CAP.   We are currently working on a way to verify manually entered results in the HIS - perhaps a pop up window???


 


James - that is a very interesting way to interpret that checklist item.   Great suggestion on getting devices interfaced whenever possible.
