Requesting POCT tests at their location

I was hoping someone would share with me their forms or process for how clinics request additional POCT at their clinic (that is already approved by your organization but they were not performing it) and request for POCT that would be new to the organization.


We are going through a changeover to LabCorp and it has been a disaster so they are looking to potentially add in more POCT in my clinics.

Here is ours for Providence Sacred Heart and our other Spokane are hospitals.

This is what we utilize.  We have implemented systems adhering to regulations only having to discontinue after performing a cost-benefit analysis after six months.   I stress the non-negotiables set by:  manufacturer to include workflow, regulatory requirements, annual user training to include non-productive time and overhead to cover training materials, reimbursement rate, volume, and/or ability to turn over service.  It helps to have the requestor perform the research prior to a formal request via committee.
