Free-standing ED/UC

I am also very much interested to speak to any of the members who have managed a free standing ED with point of care lab model.
What analyzers did you use? which ones worked which did not?
what would you do differently?
Did you ever consider having x-ray techs as POC superuser at site instead of nursing staff and if so, how did that work?

We have 2 freestanding ED/UC sites, and are in the middle of bringing up a 3rd opening Sept 1st. Setting up the first one was quite a chore; they wanted so many tests, and there aren't POC devices available for all of them (ammonia and lipase being 2 of them). We use the following: Sysmex XN500 for CBC/diff, Hemochron Sig Elite for INR, ChemSTAT for VBG, BMP, and lactate, i-STAT for troponin and BNP (also backup for ChemSTAT, Piccolo for liver panel and amylase, Clinitek for udip and pregnancy, Liat for Strep, flu, covid, and RSV, Triage for D-dimer and UDS, Inform II for glucose, Lucid Plus for ketone, also a mono kit, pH, stool occult blood, and Lifeloc for breath alcohol. It is a really difficult lift for POCT, due to all the devices, especially the mod complex ones. We have a technical supervisor to monitor, as well as a POC tech. The lab is staffed by lab assistants and EMTs; rad techs and resp techs also do testing when needed to cover, and nursing does just a few of the tests (glucose, pH, occult blood). The lab staff also processes and sends out tests to our other lab sites for tests that can't be done there, or that need follow up (like abnormal CBCs).
