How to present a Self evaluation report during inspection

I missed to submit my Infec. Mono. results to API and so a self evaluation will be my option. The report came out and my results were all acceptable. I'm pretty new with POC and still learning, just want to see how you do a self evaluation report when an inspector comes. Thank you.

Your core lab should have a proficiency testing (PT) investigation form, used for a PT failure.  You can use the same form to document your self-evaluation. 


If you cannot find one, create one with the following info:


1. A table to include your results, the group's means from API and acceptable ranges


2. Document what occurred


3. Document how you will prevent it from reoccurring


4. Signatures of testing personnel or yours (in this case), supervisor, and lab director.


Attach your support documents to this form.
