Automatic re-certification of glucose operators

The purpose of the survey is to gain insight on how POCC are handling the annual re-certification of glucose operators. The survey takes about 15 - 20 minutes to complete. 

The data collected from the survey will assist me with my Masters in Healthcare Informatics Capstone and will provide a better insight if facilities are utilizing the automatic operator re-certification or e-learning interface.
