Telcor ADT settings

Telcor users - what do you know about the details of your ADT feed? We have been having issues with pre-reg accounts and pre-reg update messages creating "open" encounters in QML. If a patient is currently say admitted to 7th floor and then a bunch of pre-reg encounters for clinics visits are entered, QML will pick the most recent open encounter and start sending results to that. 

We are considering having IT stop all messages having to do with pre-regs. I don't see why I would need these messages - I only want active encounters. Any thoughts??

Hi Danyel - we are a large system using Telcor/QML and we have all patient ADT messages going to Telcor from our LIS and have no issues with selecting the right visit - however, we use the patient account number/financial number to identify samples - which invariably makes things much easier.  Are you using MRN?

We are using MRN. Yes, using FIN would ultimately solve the issue. I know that our LIS would like to do that but it's not in the cards any time soon. 
Your barcodes on patient ID bands are the FIN? 

Correct - and it was actually this issue that we convinced hospital administration to make the change on patient wristbands (the MRN barcode wasn't getting used for any other hospital application anyway).  It ended up being not that big of a deal to change, believe it or not.  That change took rejections in QML from 40-50 a day (because of too many confounding visits on the same patient) to basically zero.  However, even if you can't do that right now, reach out to Telcor for solutions.  I have one location that because of device limitations, cannot accept a FIN, so they must use the MRN.  Telcor settings can be tweaked so that it can make the correct choice amongst multiple visits when MRN is the PID, and patients in this location typically have multiple visits around the same date.  It has been very successful for us, though it is a low-volume area.  See what they can do for you.

We do the same as James.  The majority of our sites and instruments use the account number but we have a few (like Clinitek classic and DCA ) that use the MRN.  What LIS do you have?  We have SQ and they were able to scale back the number of ADTs they got.
Our inpatients have used the account numbers since we changed our HIS to EPIC but our satellites were still using the MRN.  When we added MTE to the outpatient sites we had them switch over to account numbers.  They don't band the patient but they can print an ID label for the patient and that has the account number as the barcode.  The MR# is on there also but since the barcode is the account it made it an easier switch.  We also locked out manual entry of patient ID on the glucometer so it all fell into place alot easier and without as much griping lol.
Like James said thought I would check with Telcor and your LIS to see if they can limit the ADTs that come across depending on the A0 type.

We definitely need look at using FIN in the future. We have Sunquest also. Adonica, do you have any record of what ADT codes you blocked from Telcor? I'm actually running into the most problems getting my LIS on board so having something from someone who has done this would help! We should be able to block anything having to do with pre-registrations since we're only concerned about active encounters. 
I have talked to Telcor but I need to follow up again.
