iStat code 32 for Creatinine

Hi,

I have been seeing numerous code 32, intermittently, for creatinine cartridge lot A20331. It was first reported to me by our outpatient phlebotomy site( they have one analyzer) and then on 4 of the ED analyzers. It has appeared on 8 different meters, run by multiple people, since February. I quarantined the lot and contacted Abbott in March. They had me send a box back for investigation, and they sent back a response that they did not observe any code 32 for this lot and that no additional sites reported this code for this lot. I ran QC this morning, using this lot, and got a code 32 on three out of four meters (2 codes on level 1 and 1 code on level 3).  While it is an intermittent problem it is occurring enough to be a nuisance, so I am hesitant to put it back in use. I was wondering if anyone else was having the same issue with this lot? Thanks!

I have been getting this code Error 32 for the creatinine cartridge lot A20349. It is intermittent as well. When I called it in to Abbott, all they had me do was to run the Ceramic Conditioning Cartridge. Abbott said that sometimes the point of contacts need to be re-conditioned. Abbott stated to do this x4 and run it like a normal electronic stimulator. It will fail, but it is ok. I gave Abbott the lot number, but they stated no one else is having issues with that particular lot number. After performing this re-conditioning cartridge x4, I ran QC using this lot number and had no issues at the moment. Later on that week, caregivers were having issues again with that same error. It wasn't on all cartridges, it was just some of that same lot number. They have since gone through this lot number and switched to a new one and has not had any more issues with Error 32.

Yes!  I was also having issues with lot A20289.  I only have one meter that runs Creatinine, so I ended up requesting a replacement meter after having multiple error codes and no resolution from the CCC.  In addition to code 32, I also had several 30 and 31 codes and a random instances of 79, 43, 35 and 34.

Has that replacement meter given you any more error codes so far?

I had not actually looked at this yet, thanks for asking!  I have not seen any errors since, aside from overfill (code 30).  I'm also now on a new lot number, A21009.

Definitely sounds like a manufacturing defect.   Code 32 is not a common one.  I had several for a couple of weeks in February on CG8 cartridges but none since and none on Creatinine but we are not on this lot#.  Closest we had was A20357.  Ivy

Had the same experience with Crea Lot # A20330 on multiple analyzers. Abbott investigation revealed no issue with lot. Found a large decrease in code 32 after moving to new lot # A21006. 

I have many error codes including 32.  I ran the conditioning cartridge on the i-stat and they seemed to go away.  I did call Abbott, she told me to do that also.  Abbott sent me a new box of ACT.  I was having more issues with ACT then any other cartridge.  CG8 and EG6 both had errors as well on different instruments.  All of the errors cleared up as quickly as they came.  One week a bunch of error codes the next week with very little intervention and I haven't seen another one.  

Good morning! We have had numerous Code 24's and a few Code 32's on our creatinine lot A21030. Called Abbott but they haven't called to request a box yet.

iSTAT creat lot A20349 has 12 "code 32" so far which is many more than usual.  I have not called Abbott - it my 10+ years of experience, usually come back with " lot is performing as intended" and just a waste of time to send back a box to get this answer.  We use lithium heparin tubes so a repeat is relatively easy. 

Apparently this thread got Abbott's attention!  They called me yesterday to ask for additional information and are going to open an inquiry into lot A20289.  Patsy, I agree that the answer is too often that they cannot replicate the problem!
