hCG discrepancy

Hi all,
I wondered if anyone had an issue similar to this or has any idea why the results could be different.

One of our units got a Positive result when they did a uHCG (they repeated it) on the SureVue uHCG kit but when they sent a serum sample (gold top) to the lab the result was Negative.
• The nurse brought the urine sample down and we repeated the test on a urine hCG kit in the POCT office and the result was still Positive.
• We retrieved the serum sample in Chemistry and repeated the test on it using the SureVue Serum/Urine hCG combo kit that they use and the result was still Negative.
• We repeated the test using the urine on the Serum/Urine combo kit and it was Negative.
• We repeated the test using the urine on the Urine hCG kit we got from Micro and it was Positive.
• I did urine QC on both kits just to verify.

Any thoughts? If anyone uses these tests could you let me know what your Package Insert has for the sensitivity.

Oh my goodness!! We use the SureVue for urines only.  The lower limit is 20mIU/mL.

Side note: I don't want to hop down a rabbit hole but isn't there some cancers that will show positive on a cassette and negative in the lab chemistry instruments?

Did you try re-collecting the samples and running again?  Was it perhaps the wrong patient's blood or urine?

Hi, we also use the SureVue for urines as well.  I haven't heard of any discrepancies but we just started using this kit a couple months ago.  Could it be interference with the patients taking Biotin supplements?

The false positive could be due to the heterophile antibodies or non-specific protein binding.

Hello
We use both SureVue kits starting in 2019.  Physician office practices and ED, SDS, ENDO all use the urine only and the main lab uses the urine/serum kit. We switched to this kit from QuickVue due to biotin interference.  Choose this one specifically because per manufacturer biotin doesn't interfere.
  • Urine Kit has sensitivity of 25mIU/mL
  • Combo hCG-Stat Kit Urine - 20mIU/mL & Serum 10mIU/mL
  • Both kits use the exact methodology
I would call technical support.
