POC Troponin for ED

Hi,
My hospital wants to add a Point of Care instrument for Troponins in the ED.   Do any of you have an instrument in the ED that is used just for troponin and how does it compare to your chemistry analyzers in the lab?

At our facility we use the Abbott i-STAT for ED troponins.  It takes about 10 minutes to get a result.

WE have i-STAT to run troponin in ED.  When we had Abbott Architect in the main lab, the results are comparable.  When we got Siemens EXL in the main lab, the results are like matching apples and oranges and the ER doctors quit using the i-STAT. Our main lab  turnaround time is acceptable  so the doctors figure out that they can wait for the Main lab troponin.  They do not want serial testing but the  initial has different reportable ranges. 
High sensitivity troponin is coming out soon. 

iSTAT Troponin is pretty much the POC method of choice in these parts - it is true they do not compare well quantitatively with Siemens instrumentation (especially on the really high values), but you simply need to introduce a separate cutoff (normal/non-normal) for screening patients and caution that the two methods cannot be interchanged when serially tracking.

We use iSTAT troponin at POC in ED.  We have high sensitivity trop on the Siemens Vista in main lab. The POC is done on initial draw "0" hour only - want to quickly catch the emergent non STEMI's. The initial is also done in main lab high sensitivity - to use the chest pain accreditation term we "re-baseline" then do high sens for the 2nd and 3rd in series and all done on the units. Concordance from POC to main lab remains good.
