Nova Glucose Testing Comment Requirements

 We will be switching to the Nova StatStrip glucose meters by the end of the year.  I am working on writing the procedure for testing.  

We currently require operators to send confirmation sample to main lab if glucose result is above range of meter unless they are on DKA or Hyperglycemic protocol.  And then they would need to put a comment in meter indicating patient is on these protocols. We also require a comment when repeating patient testing.  We pull reports daily to look for compliance.  

I am curious what other facilities require and monitor.  
  • Do you require any comments for patient testing? Do you monitor comments for compliance?  What do you do if operators aren't compliant?
  • Do you require operators to send confirmation samples to main lab if above or below range of meter? And do you monitor this? 
  • Do you require under any circumstances for operators to repeat patient tests?  
  • What comments do you have available in the meter?

- We have our Nova meters set to require a comment so staff document "Meter Disinfected" after each patient use.  I report the data to our HAI committee,  they bundle it with other data (like hand hygiene compliance, isolation precaution signage, etc) and send to each manager for follow up.
 - We require confirmation of any result below 50, it's an automatic reflex built in Epic.  I was getting a weekly hypoglycemia report and sending loads of emails, but now everyone is very compliant and I pull the report monthly for the few outliers.  We recommend confirmation of anything above 450 and any result that does not correlate with patient condition.
  - We recommend selecting 'reject' on the meter and repeating the test with a new sample if the result is questionable.
- Comments available are: Meter Disinfected, Repeat Test, Procedure Error, Notify Provider (our surgery areas prefer to use that instead of documenting in flowsheet), and FreeText

We are currently using RALS middleware and the associated software is where I configure the comment codes.  Don't have very many...I want them to use the comments without searching through a long list.
  • Our hospital requires operators to add the comments to critical values <40mg/dl & >400mg/dL. Notify RN or Notify MD to comply with CAP requirements for notifying critical values to a  person that can follow-up on the result. 
  • We stopped requiring adult repeat lab draws for <40mg/dL and have gone to a treat and repeat algorithm "Treat patient immediately and retest via fingerstick in 15 minutes" an update to put into place based on the ADA guidelines.
  • Each day I review all the critical values for appropriate commenting and follow up based on the institutions nursing/lab guidelines. 
    • Monthly a glucometer compliance report is sent to Clinical Manager for review as well as to the operator.
  • Our protocol doesn't require repeat testing on critical values- we based this on the fact the main laboratory doesn't normally repeat critical values.  Operators are instructed to repeat any test that isn't consistent with the patient's condition or they are confirming a value.  RALS software is configured to hard stop results with the following comments: Operator Error, Patient ID Error, Questionable results. I follow up on this in real time.
  • Meters have free text capabilities.  I am unsure of any canned comments already installed but I'm sure there are some!

For those that use QML/Telcor, do the comments entered into the glucose meter for criticals cross into the medical record?  I was under the impression that they could not. We train operators to document in the Provider Notification area in EPIC.

Hi Sylvia,
The glucose meter comments do not cross into our medical record, but I could retrieve them from Telcor if anyone ever needed them.  The majority of operators document provider notification in EPIC, our surgery nurses like to use the "Notify Provider" comment even though they don't technically need to document anything because the meter user is the person who treats the patient.
