No bites on sharing ACT policies on my last post... 
Anyone willing to at least share their facility's target values for various procedures and/or reference ranges?

Is this ISTAT ACT or Hemochron?

We're using the Hemochron, but I'd love to see any other facility SOP on ACTs as I work on revising ours. Thanks! Sending a private message with my email : )

We use iSTAT ACT K.  Not sure reference ranges would be helpful to you.  Target values for cardiac procedures are set by our interventionists.

Actually, I'd be very interested on ranges set by anyone on any methodology. It is difficult to gather this sort of information online and it is starting to feel like the wild west of point of care tests as I talk to staff that have done these at different facilities. References and recommendations are hard to come by outside of the very basic, minimal information contained in package inserts. Anything is a huge help to my understanding.

I haven't used Hemochron in many years, but I would think that the company would publish reference ranges.  
I am getting ready to leave for the day, but if you will email me, I have references.

Using the Medtronic ACT II / ACT PLus. We validated the normal baseline ref range using a large data pool of baseline results. Vancouver General Hospital.

I am in the same boat. If you find any information I would appreciate it as well. I was told by my application consultant that the Normal range is available in the package insert. Normal Donors 113-149. Non-Heparin Patients 89-169.  I have data to prove this range for our patient population. The therapeutic range was previously set by my facility but I don't have any information regarding the Why, How or When this was adopted.
Reference Intervals

  • Normal
    • The normal baseline range for a Non-Heparin adult patient is 89-169 seconds. 
  • Critical range in Cardiology (Special Procedures) setting:
    • <200 seconds- acceptable to pull sheath
    • >400 seconds-  re-check hourly times two
    • Notify the Physician immediately.
  • Linear range
    • 67-400 seconds

VI.   RESULTS REPORTING: 
   Reference Ranges, Reportable Ranges  and Test Unit Conversions
 Reference Range:      92-181 seconds  ( arterial or venous) 
Reportable Range:      50-1000 secs 
 
VII: THERAPEUTIC RANGES 

Based on the initial study  and according to the cardiologists  practice, the following ACT values will be used  during the procedure.  
Results are immediately shared with the physician and orders are simultaneously given and carried out to maintain a therapeutic ACT per individual preference.  The results below are general guidelines only.  Every physician and every patient are different and require different therapeutic ranges. 

 | Celite  ACT |   RANGES
 | Reference Range  | 92 - 181 seconds  - non coagulated patients 
 | Therapeutic – Cath Lab  | 240-290 seconds 
 | Therapeutic –CABG | 300-400 seconds 
 | Therapeutic- For Pump procedures  | 480 -550 seconds 
 | Pull Sheath  | < 180 seconds 

We just changed out our Hemochron Response for the Hemochron Signature Elite.  This was a big undertaking that took me over a year to get switched.   We are using the ACT-LR in the Cath lab and ACT+ in the EP lab. Therapeutic Ranges for the ACT-LR Intervention are generally > or = 280 or a dictated by the physician and therapeutic Ranges for the ACT+ are reached as per physician preference by testing. 

We did put up a chart that had the new Target values but ultimately the physician makes the call. Each physicians use of and patient response to heparin are different.
