What do you do to fulfill all 6 forms of competency for moderately complex annual competency?

We use online modules in MediaLab, SuccessFactors, a form from Abbott and an in-house competency at initial access, competency form at 6 months and annual with verbal and hands on demonstration of policies, procedures, and meter use. We also monitor that the employee has run at least one patient or control every 3 months.

Just finished a JACO inspection here last week and I am located in NYS.  For i-Stat and other moderately complex testing, there now needs to be forms of competency.  Which include supervisory functions and skill.

Gina, is that for JACO or is that CAP as well? Do you have a link to the new competency standard?

We have super users on the units that do the semi and annual competencies for our mod complex testing.   Pretty much we have the users run a patient sample (using QC) and a QC sample for each mod complex platform and the super user follows through with elements based on that.
 | Superuser responsibilities for Nonwaived testing                                                                                                                                             (Accreditation regulation is highlighted and below it are our steps to meet that regulation) |
Direct observation of routine patient or simulated test performance including the following as applicable.
 | Assessment of test performance of through Proficiency Testing or Blind sample testing. (Superuser will observe operator) |
Follows Policy and Procedure
Follows standard precautions
Checks two patient identifiers
Demonstrates proper sample collection and preparation techniques
Performing a patient sample using QC as the patient sample.  Results must be within acceptable range for the QC.
Demonstrates proper sample analysis technique
Monitoring for Records ( Superuser will ensure the operator)
Evaluates and interprets patient results appropriately by showing the operator :
out of reportable range picture
Records and reports critical results as applicable -picture of critical alert values and screenshot of EPIC results
Direct observation of performance of maintenance and function checks as applicable.  (Superuser will observe the operator)
clean the analyzer
Review of test results, worksheets, quality control records, proficiency testing results and preventive maintenance records as applicable
Operator must perform 1 level of liquid QC and it must be within acceptable range
Evaluation of problem solving skills. (Superuser will show operator pictures of errors)
Operator must give appropriate resolution to errors from pictures
| Superuser will mark operator as complete in NU

Jason - We only deal with NYSDOH and JACO.  I do not know the CAP requirements on this.  However I have had the "joy" of 2 CMS visits.  CMS on both occasions compared NYS/JACO to what we had of course.

Adonica,  What state are you in and what about educational requirments?

For us the nurses had to have the 4 year BSN degree and others, such as RT's also had to have 4 year degrees to qualify as supervisory or Observer roll

We are in Delaware and yes all our super users have BSN or BSRT and 2 year experience before they can become super users.  That's a pre-req before we set them up for super user training.
