Good morning all,
I have been noticing an increase of strip errors on this lot.  Is anyone else using this lot or seeing this?  On the same line...what percentage of stirp errors do you go with  before contacting the company?

Hi Adonica,
We are not using that lot, however, our percentages for strip compliance are slowly increasing.  We usually run around 4.1%, however in Sept. we were 5.9% and last month (October) we were @ 5.3%.

In 2019 had a lot number with increased flow errors.  Reached out to Technical Specialist at NOVA and was told that anything between 3-5% error rate was considered within normal operating range.  They asked if there were specific error codes...I did a quick review and sent them a guesstimate of each. Stated "as of now we will continue to monitor this lot#". Wastage for time using that strip lot was 4.6-5.0% which is why I contacted them.

We are a small community hospital:
Monthly average of all tests performed (QC, Patient, Linearity, Proficiency) 3400
Currently monthly average no value/wastage:  3.2-3.9% 

I have noticed an up tick in errors and Octobers wastage was 3.8% which is at the higher end of our average but not outside the 5.0%.  I did a quick tabulation for November after seeing this posting  and currently at 3.9%.

We are not using that lot and fortunately haven't seen an uptick in flow errors or other issues.  We typically see around 3%,  our goal is 4% or less.

If I may ask, how are you getting your data?  I was never shown any reports from NovaNet.  The rep who was here at implementation said that the reports weren't great and that I would just use what was available in my LIS.  Unfortunately there is no such report in my LIS (Orchard).

We get ours from Telcor not Novanet.  You should try your middleware for reports if you have one.  Novanet isn't as robust on reports and things.

Our middleware is Orchard and unfortunately there is no report that exists.  But I'll investigate what, if anything, can be done.

Adonica - would you mind sharing how you pull that report?  We have Telcor and we are planning to go live with the Nova meters towards the end of January.  

Adonica, we are using that lot number. since August.  
Our Strip error  average around 2.0%. for ICU and med/surg . 
The other units are almost nil. 
The other hospital I monitor average around  1.9%
They use the same lot number we do.

If you are using NOVA StatStrip with Telcor as middleware , email me and I might be able to help you with Reports.  I was happy to have Reports I can generate at the end of the month since we had NOVA meter and Telcor.  Telcor is very good at helping set up Reports.   You just have to ask. You tell them what Report you want and they will walk you through how to generate them.  Someone has to show me great Reports, for me to know the difference between those and Telcor's. I think I got meaningful  Reports I can use .for my QA.

I used the manual method lol.  I pull a filter for the tests under that strip lot number, sort by errors and then divide to get my percentage of error.  There might be a way to get this info out of reports but I haven't tried to find it :).

I have never officially monitored strip errors but now out of curiosity I pulled my data from 2020 to present. I'm around 6-7%, so higher than you all say your goal is. 
What steps have you taken to try and reduce errors? And what errors specifically are you pulling - all of them? Bad sample, Flow error, Replace strip, etc. 

Currently using RALS middleware.  
Report parameters are the month, flag is NO VALUE (this includes patients and QC).  Captures a broader picture than just patient testing...especially if there is a concern about a strip.
Also using the report to determine if there are staff members with greater than 6 flow errors/month. Will monitor and if consistently > than 6...do a quick retraining session on performing a fingerstick.
