POC critical results

Hello, I am new to this group and this is my first posting. I a POC specialist for a few POC labs that are located in free-standing ER's (no traditional lab on site).
The first part of my question is: How do your sites deal with critical results on POC tests? Do you have the nurses repeat the test for verification? We currently have them repeat because we can't verify the result in a traditional lab in a timely manner (it would be a send out test).  
The second part is if you use a middleware system (i.e. RALS), do you hold the critical results for "evaluation" or do you let them go through? We use RALS as our middleware and have it set to hold the critical results until we see a repeat test and then we upload the results. This slows down getting the results to the provider so we are trying to come up with a more efficient way of dealing with our critical results.

We also have RALS and do the exact same thing with the critical results. They are held there until repeated. I would love to hear if there is a more efficient way to deal with the critical results. We are also working on having the POC operators document the critical results in our HIS just like they do in the lab with a "read back by" . They normally (with the Glucometer) enter a comment that transfers into RALS and then into the chart so this will add even more time onto a critical result. Not happy.

We use RALS as our middleware just for glucose testing in our Express Care Units off site.  They are Joint Commission & CLIA accreditated sites.  We do not require repeat testing of critical values unless they are inconsistent with the patient's condition.  A commment is appended to the result via the glucometer (ie Provider notified) which is time/date stamped.  The operator logs into the meter so I have all the required information, name of operator, date/time of test, providers charted response to the critical value.  These sites are only perform CLIA wiaved testing.
