PPM Skin Scraping

I have a Dermatology Clinic who currently performs PPM Skin Scraping. To my knowledge, they are only supposed to be resulting out Yeast (absence/presence), scabies (presence/absence) and ectoparasites (absence/presence). For the ectoparasites, there is no higher classification besides Scabies. If ectoparasites other than Scabies are present, the doctor/provider is supposed to only result out that ectoparasites are present. If further identification is needed, the specimen should be sent to the  main hospital laboratory or to our reference laboratory.

For PPM Skin Scraping, can PPM doctors result out anything other thing besides Scabies. I couldn't find any literature on whether anything else besides Scabies can be resulted out. Currently, proficiency is only performed on Yeast and Scabies identification.

What does your PPM clinic for Skin Scraping result out?

Hi Mai, I don't audit patient EMR records for PPMP. Neither does university (the mother ship) POC lab overseeing all nonwaived testing. But it's a good question! 
I'm off to ask the Derm folks. Will report back!

PPM Procedures:
  • Direct wet mounts preps for the presence or absence of bacteria, fungi, parasites and human cellular elements.  The regulations don't seem to limit the type of parasites that can be reported.

  • All KOH Preps

Amanda: It doesn't limit what kind of parasites they can identify, but if doctors identify other ectoparasites such as "mites", shouldn't they have proficiency testing for that specific parasite as well?

Although, proficiency testing is a substitute for patient specimens it may or may not include all possible elements or organisms.  
  • PT should be for the test system/procedure not specific organisms found in the test.
  • Is the PT being used for a wet prep/KOH (test system) or is it just for the presence or absence of scabies and fungal elements? 
Sounds like splitting hairs....hopefully more POCs will weigh in.

Thank you for your input. I was just wanting some clarification. I guess the PT is for Skin Scraping as a test system and not necessarily organisms. I am just hesitant because it is a bit weird that doctors can be resulting out any sort of ectoparasites even though they are not technically proficient in identifying to that level?
