iSTAT to iSTAT comparisons for large numbers

Looking for suggestions on working smarter and not harder!!
I manage 6 hospitals and about 119 iSTAT's.  The largest site has 60, so trying to see how others are comparing analyzers to each other 2x per year. I want to stick with using QC, and compare in batches to our "reference iSTAT analyzer". We have multiple lots of cartridges as all are delivered to main site and then shipped to the to other sites as needed, so a little bit of logistic work.

--Are you comparing all analytes on all analyzers? (Not crossing CAP numbers). Example-Department XYZ uses EG7 and Chem8, are you comparing using both cartridges on all analyzers, and what about cartridges that are not used in the department like BhCG

Trying to not break the money budget and the amount of time it will take me and one other person to accomplish, and of course be compliant with CAP
Thanks!!
Kim

Hi Kim,
Do you use the same lot for QC?  Can you pull that data from the monthly QC that your sites do and just pick a certain amount each month to compare.  We do it by analyte and on as many as we can get in 6 months and we have 1 istat that is the 'reference' so we compare it to that one.

Hi Adonica! We will be on the same lot of QC by October, currently some are on a lot expiring in Sept. I know same lot QC will make it easier. What about cartridge lot's?  Are you using all the same lot cartridges and are you testing all analyzers with all cartridges?

Hi Kim,
We did not compare iSTATs across different facilities, only within the same CAP number.  We used to run liquid QC after the CLEW Update and Thermal Probe Check on every handheld and would use that data as our iSTAT to iSTAT method comparison.

iSTAT instrument to instrument we compare the same lot of QC on each instrument for each analyte.
For the all instrument comparison, we run the same sample (willful volunteer/voluntold) on one of each of our instruments.

We do have some iStats that are at a different site/CAP number, and we do compare those to our main hospital.
So yes we compare all analytes on each iSTAT but we use the data from the monthly QC that is run so we don’t run it more than once.

Edited to add: we have 67 total iStats and 3 POC staff at 4 sites.

Note: According to CAP, quality control can only be used for method comparison studies if the control AND reagents are of the same lot number.

Reference: COM.04250 "Quality control materials for tests performed on the same instrument platform, with both control materials and reagents of the same manufacturer and lot number "
