POC INR Ranges - Enter/Edit

For those of you that use Epic enter/edit and have multiple "ranges" for POC INR results, how do you handle?  Do you use a narrative smart text?

In addition to the reference range, we have a result comment that goes out with each test listing the ranges for different therapeutic ranges, as determined by our Anti-Coagulation Pharmacists.
Result comments:
Low INR Anticoagulation Therapeutic Range:       2.0-3.0
Moderate INR Anticoagulation Therapeutic Range:  3.0-4.5  

Effective 06-27-22, note patient INR value reported, only.

Question for you.....do you report only the INR, or do you report both seconds and INR? 


That result comment, is it a narrative they have to manually add to each test or is it automatically applied to the enter/edit test?  Would love to discuss further with you about the setup.  We require they result out both the PT and INR result.

It is automatically applied to each result. We use EPIC for our LIS; our Beaker analysts built those result comments, so I really don't know the technicalities behind that. We were reporting both the seconds/INR, but our ambulatory team (clinics) made the decision to only report out INR a few months ago so we went with that for our outpatients here at the hospital as well. We use the CoaguChekXS for our POC. (We still report seconds/INR from our ACL TOPS instruments in the lab for venous samples.) It seems that patients and pharmacists are pretty much only interested in the INR anyway, and we haven't gotten any complaints about not reporting the seconds. 

EPIC added a similar comment to our results. How do you report a Coaguchek result >8 in enter/edit? Our EPIC team is having difficulty building that.
