Locking Out Users Policy

Hello all,

Do you all have it written in your policies that users have the potential to be locked of POC devices? If so, what are your requirements for this? Right now, my facility just has it written that Remedial trial training will be performed when we identify a user with performance issues and our pathologist suggested to add more to include locking out users/monitoring. 
Just look for ideas as I do not want to overload myself!

We require different users to run controls or proficiencies to maintain access.  It's a relatively new approach-my predecessor didn't.  There were a few laggards until we pulled their access.

At my previous institution, our medical Director required lock out of all operators requiring remedial training and anyone who was past due for competency assessment. This was documented in our facility level Point of Care Testing policy and re-iterated during initial training. It really helped with achieving/maintaining competency deadlines.  

Users must complete initial training (eLearning and a skill-check) prior to gaining access to a specific analyzer type at a specific facility.  They automatically get locked out if they do not complete the semi-annual and annual competencies.  The analyzers are setup to give a warning of access expiration prior to being locked out.
In smaller facilities you could potentially manage this manually but in larger facilities it quickly becomes a requirement to set it up this way in order to maintain regulatory compliance of competencies.  It's the users responsibility to maintain competency and access... just like their own personal professional licensure.

Users have the possibility of losing access through mis-use of the system as well, in this event the users manager, Lab MD and POC manager would address the situation.  Remedial training would typically be the result but more progressive actions have also taken place.  Note: This is very rare. 

We currently don't have POC middleware, but when we do, i'd agree with the remedial training, lapsed competency lock outs.
Currently, on the devices, we have it lock when QC is past due, with a grace period of the device allowing 5 Stat patients to be performed after lock out before complete lock out.
