We have several new INR devices waiting to be placed into service. In the past we would ask outpatients coming in for INR testing to allow the phlebotomist to collect a little extra blood for our studies. For each new meter we ran 3 patient comparisons. Now that Coumadin is not widely used, this process is not easy. What are the rest of you doing to validate the performance of new coagulation meters?

Definitely feel your pain there - the use of the newer anticoagulants has displaced the old standby, making this a difficult process. Other than extending your new meter evaluation process until you encounter enough eligible patients, not sure what else you could do. And probably not the answer you want to hear, but could it perhaps be the time to evaluate whether a POC INR is even necessary - since there are fewer and fewer patients on it? Would those patients have a reasonable central lab alternative? I struggled with this as well, when it was time to do 6-month POC to Main Lab comparisons or POC lot-to-lot checks. Plus, we were barely using it on patients, moving us to eliminate the test completely from our POC menu. Did my happy dance that day :)
