Competency Assessment form- nonwaived ACT i-Stat

I am trying to redo the competency form for the ACT test. I reached out to CAP and they advised for element 1 to have a super user that is in the procedure room sign off on that for me. I will be creating a seperate  "training" form I think. Otherwise, does this look ok? There isn't any maintenance or function checks on the i-stat so I wasn't sure how to show this other than having someone observe them cleaning the device itself?

Thoughts and suggestions are greatly appreciated!

Hi Jennifer,

My suggestions:
For element 4: you can satisfy by directly observing cleaning or also by observing them run QC (external or internal) since these are function checks. 
For element 5: if using QC id suggest them running it as a patient since it is supposed to be blinded. can include the acceptable range and evaluation.
For element 6: may want to include what score is required to pass

i use this for training and competency.  I have trainee do a blind sample at end of training as well as for competency at 6 months, annual and "yearly thereafter" as required. For blind sample can use QC as a patient OR whole blood.  I prefer to use whole blood esp for training  as the controls are not "red" and hard to see cartridge fill.  Easiest whole blood with expected result is a heparininzed green top (get these from other areas - creat or trop users - just go grab one of their old samples and draw up in a small syringe) expected result is >1000.  In an annual skills fair setting i will donate my own blood in a 10 cc syringe, fill a heparin microtainer, with my own blood, dispense the rest of the 10 cc syringe in a tube or urine cup.  Take a drop or 2 from the microtainer and add to the urine cup. mix well - draw up in a syringe. Run a couple of times myself - then have staff run. with iSTAT we all get pretty much same answer. I then "grade" based on Proficiency testing criteria.
