How to resolve "Order Code Translation Failure" in QML

Hello everybody

I am a new POCC and am having trouble figuring out how to resolve Order Code Translation Failures in QML. I've tried updating all pertinent info that I can get from Cerner, but it still ends up back in the interface exceptions after I try to send it. It's clear to me that I'm not updating the correct info. Any help you can give me will be greatly appreciated.

Hi Jesse,

I would call Telcor and ask for POC support. Each order code translation failure may be due to a different reason, and Telcor is excellent with guiding us through the reasoning behind it. 402-489-1207

I agree with Katherine that calling Telcor is probably your best bet. But as one possibility, at my sites, this error occurs primarily on the istat and means that a sample type was chosen that isn't set up for that cartridge/test. I just change that in QML to an acceptable sample, and away she goes!

We see these errors when there is not a correct specimen type or a specimen type selected on the iSTAT. Once the specimen type is completed the results cross over in to the hospital system.

I agree with the others that have responded. I do have one other possibility since you did not provide which testing platform this pertains. We also get this error with the Clinitek. Because our HemOc providers often just want a pH or blood reported on a urine, we have worked with Telcor to develop custom codes that will work with our Status. If the tester invents a code instead of using the provided ones, we also see Order Code Translation error.

I am also thinking now that we see this error with the ISTAT when a test is not selected and the ISTAT testing cycle is completed and the ISTAT turned itself off.

Hey thanks for the replies everyone. I did call Telcor. So I'm having this problem with ABL90 ABG results. Whats happening is that the operators (ED and ICU) are entering "mixed venous" as sample type

There is no ordering group for "mixed venous" so it gets translated to "Blank" under Qtype

And because the QType is Blank it gets held up in the Interface Exceptions. The Telcor service rep said that I could get ahold of the operator and find out exactly what type of specimen was collected and then change the QType to a value that fits an ordering group, and then it should go through when I send it. Again thanks for all the help and if any one has any other ideas I would love to hear it. I am definitely learning a lot in this new position.



You could handle this a few different ways. I'd check to see if you can disable "mixed venous" at the device level if that is not a sample type your facility uses. If it is a valid sample, you should have a mixed venous blood gas built in Telcor and Cerner. If mixed venous is being used instead of venous and there's no way to turn off this sample type at the device level, it may be possible to add another ordering group for "mixed venous" in Telcor that orders a VBG in Cerner. You'll have to work with your LIS department and Telcor to find the right solution for your facility.
